FATHER’S NAME:___________________________________________OCCUPATION___________________ Waiver Entrant agrees to release, hold harmless, and waive any and all claims against the Miss Teen World-US and Miss Teen World Organizations, pageant owners, hotel/venue property, agents, volunteers, staffs for property damage, injury, losses arising from or occurring during any pageant related events including the world finals. I am aware that my deposit of $295.00 is non-refundable if I decide to withdraw from the pageant. I have read the foregoing terms, provisions, rules and regulations carefully that is part of this application ATTACHED. I fully understand and agree to abide/comply therewith as evidenced by my signature, and parents signature herein below. You must submit a $295 deposit today, and the remaining payment of $400 before August 10, 2008. You may also pay the complete entry fee of $695 all at once if you wish. After receiving your application with deposit you will receive an acceptance letter via email and official handbook. Crown and sash will be shipped within 3-4 weeks. If you are not accepted, your deposit of $295.00 will be refunded.
1. Each delegate must be at least 13 years of age and not older than 19 by the time of the national pageant in 2008. 2. Delegates must be born female, single, never married, nor given birth to a child. She must possess beauty, intelligence, charm, leadership skills, be a successful role model for other teens, character above reproach, and a professional demeanor. 3. Official Sponsorship Fee is $695.00. The deposit fee of $295.00 is included in sponsorship fee. If accepted, contestant must obtain at least one Full Page of Advertisement ($250.00). 4. National Director has the right to cancel pageant if there are less than 20 applications. All fees paid will be refunded if cancelled. 5. Each Delegate must bring a souvenir/gift from her state for the gift exchange . 6. All decisions of the judges are final. 7. Any contestant and/or family member or guest showing unsportsmanlike behavior will not be tolerated and could result in your disqualification. No refunds will be given at this point. 8. If for any reason a winner is disqualified, or elects not to participate, her title and all awards will be passed on to the next choice of the judges. 9. The Miss Teen World-US reserves the right to change the date and location of the National pageant if less than 20 delegates. 10. Winner of the title Miss Teen World-US 2008 promises to promote the pageant by making a minimum of 6 personal appearances during their reign. Titleholders further promise to make no personal appearances or appear in any advertising and/or publications without permission from the National Director, and to return to the 2009 National Pageant to crown her successor. 11. Deadline for applications - Aug. 10, 2008. Full payment of $695.00 must be made by this date. If you have paid the $295 deposit fee already, then you need to pay the remaining payment of $400 by this date. 12. I understand that I may withdraw from the competition at any time, however, I in doing so, must return my crown and banner to the national office. Under no circumstances, and not for any reason shall I be entitled to a refund in whole or in part of any fees paid. All fees paid to Miss Teen World- US are NOT REFUNDABLE and NOT TRANSFERABLE in full or in part under any circumstances. This includes, but is not limited to all pageant fees, paid to Miss Teen World- US by anyone including, but not limited to, delegates, sponsors, and advertisers. 13. State crowns and banners remain the property of the Miss Teen World – US Pageant until after the national competition. Prizes are not transferable or exchangeable. Pageant disclaims liability for taxes, lost or stolen prizes or the action of the independent suppliers of the prizes. Any delegate who defaults on her contract must return all awards including crown and banner to the national office in the condition in which it was received and given to the first runner-up. 14. Miss Teen World -US Pageant reserves the right to modify or amend any rules and regulations, and qualifications at the sole discretion of the pageant authorities at any time. Entrant must have a valid passport and must be able to travel. 15. Delegate agrees to release, hold harmless, and waive any and all claims against Miss Teen World -US Pageant it’s owners, hotel/venue property, agents, volunteers, staffs, for property damage, injuries, losses, arising from or occurring during any pageant related event even if said property damage injury, losses results from the negligent act or omissions, of the Directors, owners, volunteers, relatives, heirs, successors, and or assigns brings any legal action in violation of any of the agreed terms or provisions contained herein above, said contestants agree to fully indemnify and defend the Miss Teen World -US Pageant, it’s owners, agents, representatives, volunteers against said action. I have read the foregoing terms and provisions carefully. I fully understand the rules and regulations and agree to abide/comply therewith as evidenced by my signature herein below.
